A comprehensive aftercare plan which includes physical and psychological needs is created prior to discharge from addiction treatment, which is recommended under evidence-based best practices.
Aftercare refers to services that help recovering addicts and alcoholics stay sober following treatment. A thoroughly thought out aftercare plan followed diligently by the patient greatly diminishes the likelihood of relapse. At Lakeview Health, we customize every aftercare plan based on a patient’s unique needs to decrease risk of relapse when returning home. Our alumni program will continue to stay in touch with recovering addicts, who choose to remain in our contact log, assisting with continued sober support and accountability.
Addiction Aftercare Begins Immediately Upon Leaving Treatment
Our Aftercare Plan offers recommendations on lifestyle changes, relapse prevention and maintenance for sobriety. Just as a diabetic needs to watch their diet and take insulin every day, the recovering addict or alcoholic needs meetings and a strong support program to maintain their recovery.
Each aftercare plan is individualized, based upon the location, medical and mental health needs, and relapse prevention plan of each addict. Addicts in treatment will create this aftercare plan with their primary therapist utilizing the input of family members, doctors, nurses and individual input.
This might include:
- Relapse prevention plan for triggers, people, places and things when returning home.
- Schedule of Alcoholics Anonymous, Narcotics Anonymous or Celebrate Recovery meetings to attend.
- Sober Living which includes: Half-way house, sober-house or returning to drug/alcohol free environment.
- Addiction aftercare groups which are local to your place of residence.
- Individual addiction counseling
- Follow up psychiatrist appointments
- Family or couples counseling
- Vocational counseling
- Continued education
- Follow up medical appointments
The goal of the aftercare plan is to aid the addict’s transition back into his or her family, work and environment. Living in recovery requires constant monitoring, support, and vigilance. Addiction treatment prepares you for a sober return to your community.
