Clinical Assessment at Lakeview Health

When an addict first arrives at Lakeview Health Systems, he or she is evaluated by doctors, nurses and therapists to begin formulation of the individualized treatment plan.  At this time, we understand that the addict will be uncomfortable, nervous and cautious about entering a detox and rehab program.  We are dedicated to providing patients with the best care and addressing the individual needs of the addict in a timely fashion.

Every one of our patients is monitored on a daily, sometimes hourly, basis as needed. To determine the best treatment plan possible, we have a seamless assessment approach from the initial assessment to aftercare planning.

Our seven assessment categories include:

  1. Pre-Admissions screening
    An Admissions Coordinator will gather patient information including medical, social, psychological and substance abuse history. This pre-admissions data facilitates the patient’s initial arrival at our detox and treatment facility.
     
  2. Admissions
    Once the patient arrives at our complex, a nurse will do a comprehensive assessment of the patient’s physical and mental status as well as review his or her medical, psychological and chemical dependency histories.
     
  3. HIV-risk screening
    This is performed for every patient.
     
  4. Medical patient history physical
    This is conducted by a medical doctor once preliminary information is obtained.
     
  5. Psychiatric evaluation and chemical dependency evaluation
    After the medical assessment, these are performed by our psychiatric team.
     
  6. Psychosocial evaluation
    Is executed in an effort to gain a clear and accurate perspective of the patient’s needs.
     
  7. Continual monitoring and reassessment of patients
    Maintaining a patient’s physical and emotional health is our priority. To meet that goal, our staff addresses changes in patients’ demeanor or physical condition in a rapid-response team mode.
