Clinical Assessment
Lakeview Health Systems is a modern, state-of-the-art detox and drug and alcohol treatment facility. We are dedicated to providing patients with the best care in a premier setting. Our multidisciplinary teams provide every patient with ongoing global assessments regardless of a patient’s level of care.
Every one of our patients is monitored on a daily basis. In some circumstances, monitoring is hourly or more. To determine the best treatment plan possible for a patient we have a seamless assessment approach, from the initial assessment to the aftercare planning.
Our seven assessment categories include:
- Pre-Admission Screening. An Admissions Coordinator will gather patient information, including medical, social, psychological, and substance abuse history. This pre-admissions data facilitates the patient’s initial arrival at our detox and treatment facility.
- Admissions. Once the patient arrives at our complex, the nurse will do a comprehensive assessment of the patient’s physical and mental status, as well as review the medical, psychological, and chemical dependency histories.
- An HIV-risk screening is performed.
- Once the preliminary information is obtained, a medical doctor will conduct an extensive medical patient history physical.
- The medical assessment is followed by a psychiatric evaluation as well as a chemical dependency evaluation by our psychiatric team.
- In an effort to gain a clear and accurate perspective of the patient’s needs, a complete psychosocial evaluation is also executed.
- Maintaining a patient’s physical and emotional health is our priority. To meet that goal, our staff continually monitors and reassesses patients, and addresses changes in his or her demeanor or physical condition in a rapid-response team mode.
