Admission Process at Lakeview Health

We encourage anyone who is struggling with alcohol or drug addiction to seek rehab admission for the help they need as soon as possible. If you are ready to go to rehab, here’s how to get started:

1. Call our treatment specialists.

If you are concerned about yourself, a loved one or an employee, call our rehab center directly at 866-704-7692 to speak to an intake specialist. He or she will answer your questions about addiction and rehab, and provide information about Lakeview Health. We are available 24 hours a day, 7 days a week to answer your call.

2. Complete a pre-admissions intake evaluation by phone.

When you’re ready to admit yourself or someone else in treatment at Lakeview Health, an intake specialist will conduct a brief evaluation by phone to gather information on:

  • Types of substances being abused
  • Length and frequency of use
  • Any past addiction treatment
  • Areas of significant distress
  • Employment and living situation
  • Other medical information

3. Provide financial information.

To make the intake process as smooth as possible, our intake specialist will ask for your insurance information and permission to contact your insurance company directly. If you wish to self-pay, just let the intake specialist know. You can also review the partners of rehab insurance we currently work with or use our online health insurance verification to expedite the process.

4. Make travel arrangements.

After your rehab admission is confirmed, make arrangements to travel to our rehab facility in Jacksonville, Florida. If you prefer, our intake specialists can make your travel arrangements.

If you plan to arrive by air, Jacksonville International Airport (JAX) is the closest airport. Let us know your arrival time and flight number, and we will arrange ground transportation from the airport. Several bus lines also serve Jacksonville, including Greyhound. We can provide ground transportation to and from the bus station.

We do not recommend that patients drive themselves to treatment, but if it is necessary, there is plenty of parking available on site. Please note that while our treatment center is in a safe area, there is no extra security for the parking lot.
